How to Add/Edit Clinical Acuity Scores from the Mobile App?
In order to add and edit Clinical Acuity Scores, please contact Civica Support to enable this on your account. Once this has been completed, it needs to be enabled for each applicable group by an Account Manager via Settings -> Clinical Acuity Scores, then ticking the box next to the appropriate team(s).
Once you have logged into the mobile app, go to view today's visits. Select one of these visits and click on the 'Record Clinical Acuity Score' button.
When the clinician identifies the patient's Clinical Acuity Score, this will be inputted on the 'Record Clinical Acuity Score button. The patient's score will then be displayed.
The selected score will be recorded and displayed on the visit. The mobile app will also show the recent score for that patient's visits.
Please note: you will be unable to check out of a visit without first setting a patient's Clinical Acuity Score - this will result in an error message being shown. In order to successfully check out of a visit, a Clinical Acuity Score must be set for that visit.
When viewing the list of visits on the mobile app, each visit will show the patient's Clinical Acuity Score.
-
When there are more than one clinician visiting a patient i.e. a double up visit, the patient will be assessed and the Clinical Acuity Score will be agreed and inputted by both clinicians.
-
When clinicians are viewing patient's visits on the mobile app that have been in the past, the identified Clinical Acuity Score will be from the previous visit.
-
When viewing a patient's visits for today, each patient will have a displayed Clinical Acuity Score from the previous visit.
-
When viewing visits in the future, it will show the Clinical Acuity Score set for that patient's most recent visit.
-
A coloured circle with the score and the time and date submitted will also be shown.
When viewing a specific visit, the mobile app will display the three previous Clinical Acuity Score set for that patient. These will show in coloured circles with the name of the staff member who submitted it and the date and time recorded.
The Clinical Acuity Score will be hidden at first and you click 'show' to reveal them. This will show additional recent visit information along with the Clinical Acuity Score.
Please note: if a Clinical Acuity Score is not submitted and the visit has Clinical Acuity Score enabled, the mobile app will not allow the staff member to check out until the Clinical Acuity Score has been recorded.
If you tap the ‘Check Out' button it will warn you by saying 'Please enter a Clinical Acuity Score before checking out’.
On occasion there will be visits which do not require patient contact e.g. a support visit to a relative or to drop off equipment. Therefore, there is an additional drop down option called ‘No Patient Contact’ for these visits.
